- Review our policies: please read our Policies & Philosophy, Must Read information, and Fees before your visit.
- Complete your forms: fill out registration, medical history, and consent forms from our Forms page, including the Registration Form and Medical History Form.
- Complete pre-visit check-in: use our Pre-Visit Check-In. Step-by-step guides are available for computer and mobile device users.
- Share your records and supplements: send prior records, labs, imaging, and procedure reports, and upload your current supplements. You can also request records from other providers using our records release authorization.
- Set up payment authorization: complete the secure credit card authorization form.
What to Expect at Your First Visit
New-patient consultations are longer than a typical GI appointment so your physician can review your history, symptoms, diet, lifestyle, medications, supplements, and prior testing in depth. You will receive a physical examination and clear next steps, which may include laboratory testing through our analytical services, procedures such as colonoscopy, endoscopy, or breath testing, and a personalized plan supported by our nutritional services.
What to Bring
- A photo ID and insurance card, if you plan to use benefits for procedures
- Prior medical records, lab results, imaging, and procedure reports
- A list of current medications and supplements
- A written list of your symptoms, questions, and goals
Insurance and Fees
LA Integrative Gastroenterology & Nutrition is not contracted with any insurance company. Analytical services such as consultations are paid at the time of service. For technical services such as endoscopy, colonoscopy, and breath testing, our team checks your benefits and helps you use insurance whenever possible. Medicare beneficiaries should read our Medicare Patient Information. Ongoing support options include our Membership Program.
Appointment Policy
Please arrive on time and allow time for any remaining paperwork. For first appointments, missed visits without notice may result in being unable to schedule future appointments, so please contact us as early as possible if you need to reschedule.

How long is the first visit?
New-patient consultations are extended so your physician can review your full history and prior testing. Our team will confirm the expected length when you book.
Do you take insurance?
We are not contracted with insurance companies. For procedures, we check your benefits and help you use coverage when possible.
Do I need a referral?
No referral is required, although physicians may refer patients using our Referral Form.
What if I live outside Los Angeles?
You may be eligible for a telemedicine consultation. Many out-of-area patients also travel to our Century City office.
Should I bring my supplements?
Yes. Bring them or upload a list before your visit so your doctor can review them.
