  • Prescription Renewals/ Refills [See Details] (when appropriate & applicable)
  • Prescription for Controlled Substances (when appropriate & applicable)
  • Assistance with Prior Authorizations (PA) (no guaranteed outcomes)
  • Assistance with Appeals when a PA is denied (no guaranteed outcomes)
  • Coordination of Care with other Physicians
  • Coordination of Care with Pharmacists for TPN/PPN (Intravenous Nutrition)
  • Coordination of Care for Homecare Services
  • Coordination of Care with Dietitian for Enteral Feeding
  • Request Concierge Services – discuss with physician directly
  • Request Labs before a visit (honored up to twice a year)

MEMBER SUPPORT PROGRAM

Member support is essential for ongoing therapeutic monitoring, scheduling accommodations, and responding to patients’ inquiries.

A few things you should know:

  • Our Member Support Fees are increasing.
  • We offer four tiers of Member Support that you can select based on your needs and the frequency of use of our services.

LEVEL 1: CONCIERGE AND ELITE MEMBER SUPPORT PROGRAM

  • Fee is individually customized. Please send a request.
  • Ideal for patients who need direct access to the physician.
  • For patients who would like a physician as their “Personal Care Coordinator” (PCC)
  • Eligible for house calls (when there is medical justification and proximity allows).
  • Minimum commitment: 6 months.

LEVEL 2: ENHANCED MEMBERSHIP — 6 OR 12 MONTHS — PROGRAMS & INCENTIVES

  • If you’re enrolled in this Member Support Program, you’ll receive an additional $50 discount on your visit fees (for visits of 30 minutes or more) and will be eligible for a variety of additional benefits.
  • More flexible scheduling and dedicated service.
  • Purchase your membership before January 31, 2026 to lock in the 2023 fees.
  • Current members: Renew before your membership expires to maintain the 2023 pricing.
  • One-time offer: Purchase your membership before December 31, 2025, and get an additional 10% discount.

LEVEL 3: BASIC MEMBER SUPPORT PROGRAM

  • A minimum of $299 annual payment is required to receive basic services and medication refills.
  • Up to 4 visits a year.
  • Up to 4 messages a year.

LEVEL 4: NO MEMBER SUPPORT

  • You may request physician-consulting services only.
  • A $75 Cost of Business Adjustment Fee will be added to the visit charge.
  • A 3.9% credit card processing fee will apply.
  • A $100 fee for submitting prior authorizations to insurance (medication, imaging, procedures, etc.)
